As a newly formed contract therapy company, our name is new to our industry but our senior management team members each have over 30 years of therapy and long-term care experience.

Choice offers programs for residents who demonstrate a medical need and can achieve increased independence through intervention as defined by Medicare and state practice acts. Services are designed to integrate nursing care, activities, dietary and other departments to enhance the dignity and functional skills of each resident.

Our goal is to make decisions that benefit our customers by enthusiastically providing superior and fiscally responsible services and responding quickly when solving problems or pursuing business opportunities. We provide daily oversight of the care provided in our facilities. This is achieved by assigning each site a dedicated Director of Rehabilitation (DOR) to be a part of the facility team. Our DOR’s are supported by a Regional and Corporate Team including:

INFORMATION MANAGEMENT SYSTEMS
Our information management system (Casamba Smart) is installed on computers provided by Choice Rehab. This software enables the DOR to plan treatments, verify delivery of planned services, track the actual minutes and CPT codes of the therapy services and provide billing departments with concise legible reports of services. Choice also utilizes Casamba to create a set of core facility reports as well as customized reports as requested by key facility administrative and billing team members.
